O'Romeo marks Bhardwaj's third collaboration with Kapoor.

The duo has previously worked together on critically acclaimed films such as Kaminey (2009) and Haider (2014), both of which showcased Kapoor in some of his most celebrated performances.

The film draws from the story of Hussain Ustara, a Mumbai gangster who famously feuded with underworld don Dawood Ibrahim, as mentioned in Hussain Zaidi's book Mafia Queens of Mumbai, although the filmmakers have not confirmed if it is directly based on his life.
