71-year-old Mrs. Vimal Shinde (name changed) from Nerul was suffering from hypertrophic obstructive cardiomyopathy (HOCM), a serious heart condition for the past few years. Symptoms such as severe chest pain, shortness of breath, severe fatigue and difficulty walking made his daily life extremely difficult. Despite all the medicines, his condition did not improve. Even minor household chores or walking short distances became challenging for them.

Dr. Keshav Kale saidthe patient had high blood pressure, diabetes and hypertrophic obstructive cardiomyopathy (HOCM), an inherited (hereditary) heart disease. in which the heart muscle becomes abnormally thick and stiff, and the thickening of this muscle causes a blockage (obstruction) in the way blood mainly exits the heart, making it difficult for the heart to pump blood throughout the body. It can cause chest pain, shortness of breath, dizziness, irregular heartbeat, fainting, heart failure or sudden death.

Investigations such as echocardiogram, angiography and cardiac MRI revealed severe obstruction in the left ventricular outflow tract (LVOT) of the patient’s heart with a pressure greater than 100 mm Hg. This obstruction persisted despite all medication. Initially, it was considered to do ‘septal myectomy’ open heart surgery. But considering the patient’s age and other ailments as well as the risk of major surgery, this option was very risky.

About this Dr. Kale saidthe patient’s condition was critical. So we opted for alcohol septal ablation, a rare and technically very challenging treatment. This procedure was done for the first time in Navi Mumbai and Raigad districts. In this treatment, which is done under anesthesia, a specific artery that supplies blood to the thickened heart muscle is found and a controlled amount of 95 percent alcohol is injected into it. This reduced the thickness of the muscles in the area and helped improve blood flow.

A temporary pacemaker was also implanted for extra safety during treatment. After the procedure, which lasted for about two hours, it was observed that the pressure of the blockage in the heart was reduced by almost 70 percent. The patient’s condition improved significantly without any serious complications. After just two days of treatment, he felt relief and was discharged home from the hospital three days later.

The patient’s family said that the mother was having a lot of difficulty even doing simple things like walking, climbing stairs. We were apprehensive about the risk of open heart surgery. But after the successful treatment at Medicover hospital, now the mother can breathe freely and her condition is improving day by day.
