  • Reporting a potential illness is very simple. The process requires filling in the following details:
  • Personal Information: Name, mobile number, age, gender, address and occupation of the informant.
  • Description of the incident: The date, location, and brief description of the illness or health problem.
  • Evidence (if available): Photographs or other relevant documents to the incident.

Verification (OTP): After entering all the required information, the information is directly sent to the relevant health officials once the verification is complete via an OTP received on your mobile phone. Officials can also contact you for additional information if needed.
