Bureau Prayagraj- Divisional Commissioner conducted a surprise inspection of Swarooprani Nehru Hospital along with Additional Director Health Dr. Rakesh Sharma, CMO Prayagraj Dr. Arun Tiwari, CMS SRN Hospital Dr. Neelam and other concerned officials. Emphasis was laid on maintaining standard hospital protocol and antiseptic conditions in the trauma center. During the inspection, deficiencies were found in cleanliness, infection control, maintenance of medical materials and quick response arrangements.

*During inspection of various wards and OPDs and interaction with patients, complaints about medicines and tests being conducted from outside came to light. Prescriptions for outside medicines and test reports were also presented by the patients. Shortcomings were also found in sterilization of equipment, bedsheets, biomedical waste segregation and cleanliness system*

The Divisional Commissioner directed CMS to get all the arrangements repaired as per the prescribed protocol and SOP within four days. Also, a detailed investigation report has been sought from the Additional Director Health. The report will be sent to the government for action against the officers/employees found guilty in the investigation and surprise inspection will be conducted again after four days.

Divisional Commissioner Dr. Lokesh M. today conducted a surprise inspection of Swarooprani Nehru Hospital along with Additional Director Health Dr. Rakesh Sharma, CMO Prayagraj Dr. Arun Tiwari, CMS SRN Hospital Dr. Neelam and other concerned officials. He first inspected the Help Desk, Trauma Triage Room and Ward No. 01 located at the Trauma Center. The cleaning system in the Trauma Center was not found to be as per the expected standards and the cleaning work was being done at the time of the Divisional Commissioner's arrival. The patient care and quick response arrangements in the Trauma Center were also not found satisfactory and the arrangements were found to be very lax and slow.

The prescribed SOP was not followed in the maintenance of medicines, injections and other medical materials in the trauma triage room. Deficiencies like dust and dirt, lack of proper segregation of biomedical waste and lack of proper cleaning of forceps and other equipment were found on the spot. The liquid used for cleaning the forceps was also found to be old and the antiseptic system was not up to standard. Dirt and blood were found on some bedsheets, which could cause spread of infection.

The Divisional Commissioner took information regarding the arrangements from the resident and intern doctors present and said that a large number of economically weak and poor patients come here for treatment, hence it is everyone's responsibility to provide them clean, safe and standard medical facilities.

Even after inspection of the minor OT/dressing room near the Trauma Centre, the cleanliness system was found to be poor. The healing condition of an open wound of a female patient was not good and was found unattended. The Divisional Commissioner instructed the concerned medical staff not to be negligent in the care of patients.

During inspection of the X-ray room, the cleanliness system was found to be very poor. Dust and dirt, broken chair, scrap material in carton and helmets of staff were found around the X-ray machine. Unnecessary items were also found kept around the machine. Many old slips were also dumped at one place. The Divisional Commissioner sternly reprimanded the concerned technician and directed for immediate cleaning, removal of scrap and unnecessary material and to keep the entire X-ray room organized as per the prescribed SOP.

Divisional Commissioner interacted with the patients admitted in Male Ortho Ward No. 02. Many patients told that they are given prescriptions to bring medicines from outside. The patients also showed slips and reports of X-rays and other tests conducted from outside. After seeing these slips and reports, the Divisional Commissioner interrogated the concerned officers and CMS. The doctors present told that many essential medicines are not available to them by the corporation, due to which many patients have to be sent from outside to get medicines.

During inspection of ENT OPD, the instruments, forceps and other materials used for examination were not found adequately sterilized. The bedsheet was dirty and other materials including cotton/gauze were also found in unhygienic and untidy condition. The Divisional Commissioner called the residents and concerned staff present there and said that patients come to the hospital with great expectation and confidence. Therefore, it is everyone's responsibility to ensure complete cleanliness, infection control, proper sterilization of equipment and quality treatment.

He clarified that after four days the arrangements will be verified by conducting a surprise inspection of the hospital again. In view of the shortcomings revealed in the inspection and the facts presented by the patients, a detailed investigation report has been sought from Additional Director Health Dr. Rakesh Sharma. Whatever officers/employees are found guilty in the investigation, the investigation report will be sent to the government for action against them as per rules.
