First, you need to visit the Ayushman Mitra help desk at the hospital. Inform them about the loss of your card. You must also provide the operator with your registered mobile number, which must be the one linked to your Ayushman card.
The operator will identify you using this mobile number. Once identified, you can request free treatment without the card.
If the operator doesn't help you, you can also file a complaint with the Ayushman Yojana helpline number 14555.

How to Create a Card?
Step 1 - First, visit the nearest CSC center near your home.
Step 2 - Here, you'll need to meet with the desk operator to have your eligibility checked.
Step 3 - Once your eligibility is verified, your submitted documents will be verified.
Step 4 - You'll receive your card shortly after submitting your application. You can download and encrypt it. This way, you can easily create your card.
